Availing of our services is easy.  Anyone can make the referral to Holy Cross Hospice.  We just need basic information on the patient to get started with the process.  We can receive this information by calling us at (480) 718-5697, or by filling out the simple Contact form on the right hand side of this page.  A hospice nurse will then speak to you about hospice services and how to enroll if hospice is right for you.
